A review of Two Staff Records at the time of the visit inidicated that one record did not include a current physical exam for ne educator.
A review of Emergency Evacuation/Fire Drill logs was completed at the time of the visit and indicated that the program did not have documentation of conducting drills for the months of October, November and December of 2025.

Upon a review of five Individual Health Care Plans (IHCP's), the following was identified: -Three IHCP's were not current and complete.
The program did not have a medication onsite for one child who had been prescribed three medications.
Upon a review of three Children's Records at the program who have been prescribed medications, the following was identified: -Two children's Medication Consent Forms were not current or complete for prescribed (REDACTED). This means the program does not have parental consent to administer medication to two children in the event of a potential medical emergency.

Upon a review of the BRC Navigator system, the following was identified: -One educator has a "pending" determination and fingerprints were processed in 2/2020. This educator's BRC needs to be renewed. -One educator is deemed suitable but is affiliated with the Supervised Remote program. This educator's BRC needs a suitability transfer to the correct program. -One educator is deemed suitable but is affiliated with the Coombs program. This educator's BRC needs a suitability transfer to the correct program.

Upon a review of staff BRC's in the BRC Navigator, one staff member is due for renewal.
The Staff Schedule posted listed the same educators at two site locations: Coombs and Quashnet. Each site must list consistent staffing assigned to consistent groups of children.
Upon a review of three Children's Records at the program: -Two did not have evidence of a complete Individual Health Care Plan. Please ensure you are indicating the name of the educators trained in the child's medical condition and who trained those educators on the IHCP Form.

Upon a review of three Children's Records at the program with Individual Health Care Plans, the following was identified: -Two Children's Records did not have evidence of a current and complete IHCP.
Upon a review of medication stored at the program, one child's prescribed (REDACTED) was not onsite. The Site Coordinator and educators were aware of this and stated during the visit that the program has already reached out to the parent to bring the medication onsite.
