One of the reviewed IHCP's did not document a parent signature. Additionally, the document stated that the child required {REDACTED} for an allergic reaction, but {REDACTED} was not stored for the child on site.
11 {REDACTED} and 3 teachers were operating out of one classroom at the time of EEC's visit. The program did not 'split' into compliant ratios for several minutes. Only when the Program Director / Licensee arrived to the classroom, did the group divide. 2:9 is the compliant, EEC, teacher to {REDACTED} ratio.

Emergency medications readily available
Some of the reviewed staff records did not document {REDACTED} immunity; at least two of the reviewed staff records did not document evidence of a current physical exam.
Staff orientation training dates, {REDACTED} immunity, physical examination date and BRC suitability dates were not documented for several staff onto the staff records checklist.
One staff member did not have current suitability as documented in the EEC BRC Navigator System.
The First Aid Kit was not labeled and / or it's location posted in one of the reviewed classrooms. Additionally, the kit did not store a gauze roll, CPR face shield and / or disposable ice packs.
Several lunch boxes were stored on top of the changing table in one classroom. Please refrain from this practice for sanitary reasons.
One of the reviewed IHCP's documented the use of {REDACTED} for allergic reaction, however, the medication was not stored on site for the designated child.
