An accessible drawer in the bathroom contained a hair dryer with a cord longer than 6”. An accessible drawer in the kitchen contained a vegetable peeler with a sharp edge and items such as binder clips that present a choking hazard.

An electric device was unplugged but included a long electrical cord that poses a risk to children.
Items with cords were found in an accessible drawer in the living room. A book shelf in the computer room is not secured to the wall and poses a tipping hazard to children.
The door frame into the utility room was observed to be splintered. An electrical outlet in the kitchen did not have outlet covers, leaving it accessible to children.
Exits are clear of obstructions but are not marked with an exit sign, as is required.

Toothbrush storage
The Educator has one surface that can be disinfected, but is not using another surface. Discussed that the second surface must be disposable, individual to each child, or also able to be disinfected.
There was one approved assistant, Deborah Murphy on the premises. There was another person on the premises, Samantha Hughes who was caring for the children. Ms. Hughes has an application in process with EEC but has not had approval yet as an assistant. Discussed that Ms. Hughes cannot be in caretaking role with the children until she has EEC approval.

Records for 11 enrolled children were reviewed. Two children did not have enrollment packets. Two children needed Annual Update forms. One child needed proof of current physical, immunizations, and lead screen, and also was missing a signature on the General Permission section of the enrollment packet.
The Educator did not have proof of current CPR and First Aid certification. The only certificate available for review showed that CPR had expired in August 2018, and First Aid expired in August 2019. The Educator did show Licensor that she had completed the online portion of the CPR training on 10/20/19.
The Educator gave Licensor a piece of paper that she printed during the visit. The paper did not have all of the EEC required information for the evacuation drill logs.
The first aid kit did not have Emergency Cards for all enrolled children.
There was a person on the premises who has not received EEC approval yet. She is in the process of getting approved as a Regular Assistant.
At least 7 children needed progress reports. There was no proof of the progress reports.
The Educator did not have a medical form for the the Regular Assistant.
The toothbrushes were all stored in one cup, touching each other.
There were no exit signs on the doors that the Educator stated she uses to evacuate the program.
There was a drawer in the kitchen with a child latch that was not working. The drawer had plastic bags in it, making it a suffocation hazard.
There was an infant sleeping in a propped seat that was placed in the porta crib. The infant was wearing a bib. There was also a bottle between the baby’s legs and a small blanket on the legs, as well.
