The following was observed during the visit to be hazardous to children: Four ceiling tiles are stained and the wall around the hose spigot in the left side bathroom stall has peeling paint surround it.

During the time of the visit, the attendance in TODDLER was not accurate. Three children were present but four children were listed as being present. The educator in the classroom updated this when addressed by the licensor.

The following was observed to be accessible to children: -In TODDLER A, the blue and yellow table and matching chairs were rusting a the bottom and baggies of baby wipes were in bins on a shelf next to the changing table. -In TODDLER B, baggies of baby wipes were in bins on a shelf next to the changing table, the wooden shelf system had a hole in the back and sharp pieces poking out, and the black storage bin where the bedding is kept was dusty inside. The baggies have a warning to Keep out reach of children. In PREK, two cans of shaving cream were on a low shelf and contain the warning "Keep out of reach of children". The cans of shaving cream were moved during the visit.

This investigation inspection recorded no violations or advisories.
The following was observed to be accessible to children: -In TODDLER A, a bookshelf was not secure and tippable. -In PRESCHOOL, a pair of adult scissors was sitting on a low shelf and a bookshelf was not secure and tippable. The scissors were relocated during this visit. -In PREKINDEGARTEN, three shelving units were not secure and tippable. -In the bathroom next to Prekindergarten, rust was visible on the bottom of the stall door. -In TODDLER B, the toy kitchen set was not secure and tippable, the crossbar along the bottom of the sensory table was missing a screw and could pinch a child's finger if they move it, and the fall zone for the soft climbing blocks (height 18 inches) did not have appropriate ground surface material. -In INFANT, two ceiling tiles were stained. -Throughout the program, the ceiling vents had visible dust.

In the Infant Room, foam covering on a shelf was loose; in the hallway speaker wires and phone wires were draped in a manner accessible to children; in PreK the trash was not covered and radio wires were accessible to children.
On the playground fencing, screws were not covered, and were accessible to children; railroad ties (used to outline playground) had sharp unprotected corners.
This renewal - full review visit inspection recorded no violations or advisories.
Bathroom stools were dirty, one round carpet was dirty, the soft climbing structure in Infant/Toddler was torn/frayed, excess old tape on the bathroom door was accessible to children.

A bottle of peroxide, plastic bags, adult scissors, two cans of shave cream were accessible to children in classrooms. In the hallway area where strollers are stored, an unsecured water cooler and 5, five gallon jugs of water were unsecured and accessible to children. In this same area was a microwave and a toaster over, both of which were plugged in. Although the strollers blocked the area on the day of the visit, when the strollers are in use these hazards are accessible to children. In another hallway (main hallway) were 3 rolls of paper, approximately 3.5-4 feet high, unsecured; a metal, 4 foot high paper roll holder was unsecured. Two sharp hinges were accessible to children on a shelf unit. The area adjoining the Pre K Room, which is one of the directors office spaces, had the following hazards accessible to children: adult scissors, two computer monitors unsecured and one computer modem/processor was unsecured
The following was observed to be accessible to children: The bottom wooden step leading from the lower outdoor space to the higher outdoor space has a piece of wood splintering up on the top left corner. Children could pinch their fingers or trip on it.
One child's EpiPen expired (REDACTED).

A review of four staff records was conducted and found: -Two staff records were missing bi-monthly observations. The last ones were dated 3/7/2023. -One staff record did not have a current physical. The last one was dated 6/30/2021.
The following was observed to be accessible to children: -The plastic kitchen set was not secure and tippable.
A review of five Individual Health Care Plans was conducted and found: -Two children's IHCPs were not completed. Both plans were missing the side effects of treatment.
A review of all emergency medications was conducted and found: -One child's physical indicated he/she is prescribed (REDACTED) however, the (REDACTED) has not been provided by the parents.
A review of the evacuation drill log indicated that no fire drill was conducted in April, May, June or July of 2022.
