The following was observed to be accessible to children: In PRESCHOOL, the boombox cord hung loosely between the boombox and the electrical outlet. This cord is a strangulation hazard.
A child with (REDACTED) in the Early Childhood program did not have an IHCP. Two IHCPs for children in the school age program had incomplete IHCPs. One IHCP was missing potential side effects and consequences if treatment are not given and the second IHCP was missing the medical treatment and side effects of treatment.

Individual Health Care Plan (IHCP)
Four child records were reviewed the the Early Childhood program and found one child record was missing a current doctor asthma action plan (expired 1/7/2026). Four child records were reviewed the the school age program and found one child's one child record was missing an allergy action plan.
Two staff records were reviewed for the Early Childhood program and found one staff record was not complete. It was missing bi-monthly observations since September 2025.
The following was observed in the Early Childhood Program: In PRESCHOOL, four children's water bottles were not labeled with their names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
The emergency chronic medical conditions posting for the school age program was not complete. It did not include the emergency medications.
The following was observed during the visit: the surfaces of the furniture and window sills were dusty from movement and construction. The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions.
The Building/Fire Inspection is current however not yet complete as it is missing the use group code. The Licensee stated the Building Inspector is expected to correct this on Monday 8/25.

Medication
One staff member (hired in 2022) does not have current First Aid certification (expired 12/20/2024).
Three staff records were reviewed and found all three records are missing bi-monthly observations. One has not had observations since April 2024, one since January 2024 and one since February 2024.
Five child records were reviewed and found: One child's record was missing a consent form for an antihistamine and one child's record was missing a current FA/CPR consent form (expired 3/1/20218).
The narrow wall to the right of the Center Director's office has multiple holes and exposed plaster.
In the THEATER room, the evacuation procedures were missing from the entrance to the hall that leads to the front door.
The two side gates that block cars from entering the outdoor space behind the building were open.
Five child records were reviewed and found: -One child record did not have a current allergy plan (REDACTED) and did not have an IHCP. -One child's record had one medication consent form for two medications and did not have an IHCP. The two medications each need their own consent. -One child’ record does not have an IHCP.
Two staff records were reviewed and found: -One staff record was missing a physical and immunizations. -One staff record was missing evidence of completion of EEC Essentials.
The following was observed to be accessible to children: -In the THEATER room, wires hung loosely from the front of the piano and loosely from the TV on the right side wall.

A review of the staff records indicated that the Program Director did not have a signed verification of work experience or evidence of their immunization records.
The program did not post written emergency evacuation procedures by an exit door in the Main Room. This was previously cited on 1/25/2023. Please submit to the Department a plan to ensure written evacuation procedures are posted by all exit doors at the program.
A review of two children's records indicated that three Individual Health Care Plans were incomplete. This was cited on 1/25/2023. Please submit to the Department a plan for obtaining these required documents.

Emergency medications readily available
There were no written emergency evacuation procedures posted next to three exits in the main room.
A review of two children’s records indicated that: -Three Individual Health Care Plans (IHCP) were incomplete and did not include authorization from a licensed health care practitioner.
A review of a child’s record indicated that the child had (REDACTED); however, their Individual Health Care Plan (IHCP) stated that no medication was needed at the program. There was no confirmation from a licensed health care practitioner and the program did not have medication available to the child at the program.
Although a list of children's medical needs/allergies was posted it was not a current list (dated 8/22/2023). The licensee must post the following information in an area easily visible to parents, educators and visitors: in a manner that protects the privacy of each child: 1. a list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong; and 2. a list of allergies and/or other emergency medical information provided by the parent for each child.
The evacuation drill log was reviewed and found: -Drills were not documented in December 2023 or January 2024.
The following was observed: The emergency backpack containing the children's emergency medications was left inside the building when the group went outside. The bag was retrieved during the visit.
