Medication
The IHCP for child with Asthma was not complete. Missing doctor and parent signature. The IHCP did not say the child had two medications, or when to administer them.

Individual Health Care Plan (IHCP) - R Apr 2025
Children’s files missing progress reports and signed transportation plan. Child with a IHCP was missing Medication Consent form for second medication, the IHCP was not completed and signed by parent and doctor.
The medication observed did not have a prescription label or box. There were two inhalers, two different medications. The inhalers stay in the child’s back pack and travels with the child.
