The following were observed and accessible to children: TODDLERS - Two shelves were not secured and tippable; the edges of the circular zebra rug were curled, which could be a potential tripping hazard; the bottom of the white door leading outdoors had cracks. PRESCHOOL - The bookshelf and play kitchen were not secured and tippable. CHILDREN'S BATHROOM - Two toilet plungers were located in the smaller stall; there was a cracked floor tile in the bigger stall.
The rock climbing structure was not positioned in an appropriate use zone with an adequate depth of ground surfacing to prevent an injury.

The Building Inspection expired on June 30, 2021. The Licensee stated the Lakeville Building Department is behind in conducting inspections.
The following was observed and accessible to children: PRESCHOOL PLAYGROUND - There was a plastic bag in the back corner; a piece of the black border was sticking out of the ground, which could be a potential tripping hazard.
The following was observed and accessible to children: In the black top area leading to the outdoor space, there was a can of Flex Seal spray with a label warning indicating "keep out of reach of children"; an upright metal ladder with a loose yellow rope and there were large planks of wood on the ground.

The following were observed and accessible to children: PRESCHOOL 1 - Next to the hand washing sink, there were two electrical cords approximately six inches long draping. HALLWAY - On the fence border wall, the top electrical outlet did not have a safety cover. TODDLERS - Next to the outdoor exit, the metal baseboard was not secured to the wall; exposing a sharp edge.
It was observed that an evacuation procedure and diagram were not posted next to the hallway exit.

It was observed in Preschool 1, in the Block area, there was chipped paint. A review of the Lead Inspection indicated that the program is in compliance.
A review of the Evacuation Drill log indicated that a drill was not conducted in the month of December 2019.
It was observed in Toddlers, there were books and a bucket of blocks on the changing table.
The following were observed and accessible to children: KITCHEN (through the open "Employees Only" door) - there were bottles of Clorox, Lysol spray and a box of Cascade. STAFF ROOM - To the right of the entrance, there were vacuums with electrical cords approximately six feet long. PRESCHOOL 1 - Next to the storage unit near the outdoor exit, there was a confirmed staff purse on the floor. TODDLERS - In the unlocked bottom cabinet, there were two shaving cream cans that indicated to "Keep out of reach of children".
