Health and Safety
One child's Individual Health Care Plan expired on 8/31/2023. The written parental consent and the licensed health care practitioner authorization shall be valid for one year unless withdrawn sooner. Such consent and authorization must be renewed annually for the administration of medication and/or treatment to continue.

The staff schedule did not have one of the current educators assigned to supervise on the staff schedules.
The Site Coordinator and Assistant Group Leader were outside on the playground with 16 school age children. One child walked into the cafeteria(licensed space) and sat down to play with a manipulative. The child played alone for approximately 3 to 5 minutes. The educators were unaware the child left the playground. The educators had to be informed that the child left the group. Educators must exercise appropriate supervision of the children in their care at all times.
