A review of medication documentation indicated that one did not have a written medication parental consent form.
A review of medications indicated the following: 1) One was not stored with its original container with prescription label affixed. 2) A secondary medication listed on an IHCP was not provided to the program.

A review of three children's records indicated the following: two did not have evidence of a current transportation plan, one did not have a signed first aid and emergency care consent by the parent, and one did not have a dated progress report and verification that the child's parent received a copy.
During the visit, it was observed that the play kitchen and play doctor set were unsecured and tippable in the PRESCHOOL classroom.
During the visit, it was observed that one child's allergy information was not posted in a confidential manner in the INFANT/TODDLER classroom.
A review of the evacuation drill log indicated that it was not up to date.
A review of one child's IHCP indicated that there was no evidence of a parental medication consent.
During the visit, it was observed that the bathroom sink cabinet was unlocked and contained canisters of shaving cream, feminine products, and disposable glove boxes.
Toxic Substances
During the visit, it was observed in the INFANT/TODDLER classroom that children's sippy cups were not labeled with the child's name.
A review of three staff records indicated the following: Two did not have evidence of a current EEC PQ registration and EEC Essentials training completed, one did not have evidence of a current physical examination, and one did not have a signed and dated staff orientation completed by the educator.

During the visit, three staff files were reviewed. One file did not include a current physical. Two files did not include evidence of immunity to Measles, Mumps and Rubella. One staff members CPR and First Aid certification was completed online only. EEC does not accept CPR or First Aid certification that does not include demonstration of skills to a certified instructor. Staff observations were not conducted bi-monthly as required by EEC.
During the visit, two staff files were reviewed. One file did not include documentation showing completion of the 12 EEC Essentials trainings.
One child's Individual Health Care Plan was expired and did not include all of the required information. If the child is no longer attending the program, medication should be removed from the classroom and returned to the parents.
During the visit, the Evacuation Drill Log was not available for review. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.

The water temperature was above 120 degrees in the children's bathroom.
The playground did not have the required 6 inches of shredded rubber under and around the fall zones of the climbers, slides and swings. Tree roots were protruding upward, causing a tripping hazard. In addition, the Little Tykes slide did not have the required six feet of fall zone.
During the visit, three staff files were reviewed. Two files did not include evidence of immunity to measles, mumps, and rubella. Observations of educators working with children were not conducted bi-monthly as required.

The garbage barrels in both classrooms did not include a cover as required. There are two stained ceiling tiles in both classrooms.
The following hazards were observed on the Toddler Playground. Two slides and the red swing did not have an adequate depth of impact absorbing material under and around the fall zones. The two slides were located approximately four feet from the fence and did not have the required 6 feet fall zone.
One evacuation crib was being used to store classroom toys. The evacuation crib must only be used for its intended purpose.
Evacuation drills have not been conducted monthly as required. The evacuation drill log did not include the route taken and effectiveness of each drill.
During the visit, toys were observed in the handwashing only sink.

Emergency medications readily available
One child's Individual Health Care Plan includes the administration of (REDACTED). This medication was not on the premises. Therefore, it was not available if needed. Another child's Individual Health Care Plan includes the administration of (REDACTED) as needed. This medication must be on the premises when the child is in attendance unless noted otherwise by the child's physician.
Two children's Individual Health Care Plans did not include all of the required information.
