1) The children's record checklist reflected several children with expired physicals. 2) The records for two children lacked CPR/FA parental consent forms, transportation plan and immunization records. One record also did not include evidence of a current physical examination. Please ensure that all children's records are current.

At the time of the visit, a child's EpiPen was not in the containers in which it was originally dispensed and with the original labels affixed.
On 12/5/23, the program was cited for not having a staff record available for review during the visit and for a staff record not containing evidence of current physical and MMR immunity. On 12/18/23, the program submitted a corrective action plan stating, "1) the staff's folder is now in the file cabinet at preschool". "2) It has the current physical and MMR immunity. During today's visit, the staff record in questioned was not available for review and a second staff record still contained no evidence of current physical and MMR immunity. The program did not adhere to the corrective action plan submitted on 12/18/23.
