This renewal - monitoring visit inspection recorded no violations or advisories.
At the time of the visit there were boxes stacked up in the hallway to the gym and in the gym creating a potential hazard. At the time of the visit there was a gate at the half wall in the wobblers room that was loose. At the time of the visit there was felt under the easel that was not flat and was a tripping hazard.

At the time of the visit there were boxes stacked up in the hallway to the gym and in the gym creating a potential hazard.
At the time of the visit a child in the preschool classroom wiped his nose and did not wash his hands. At the time of the visit a staff in the toddler classroom wiped a child's nose and used hand sanitizer instead of washing her hands with soap and running water.
At the time of the visit one [REDACTED]child's [ did not have a prescription label attached nor did it have the proper completed consent forms for that child.

Fall zones adequate depth of impact absorbing material
At the time of the visit climbers observed in classrooms did not have gym mats by the steps or the slides to absorb the impact if a child were to fall. At the time of the visit the carpet in the toddler classroom was in disrepair. At the time of the visit the area carpet near the couches in the older toddler room had visible stains on it.
At the time of the visit there was a purple car in the fall zones of the red and yellow climber. There was a green car in the fall zone of the bike merry go round. At the time of the visit the yellow and red slide did not have the six foot fall zone on all sides of the piece of equipment.
[REDACTED] for a child did not have a [REDACTED] signature or date on it.
Staff in a toddler classroom were observed not washing her hands after wiping a child’s nose.
Diaper changing mat in both toddler classroom were observed with coats and an IPad on them.
