Licensor randomly reviewed 3 of the children's records and observed the following: ** Record 1 **- no medical information (physical or immunizations) ** Record 2 **- no evidence of lead testing ** Lucas **- out of date physical and immunizations and lead testing.
Licensor inspected all of the licensed space and observed the following: ** BASEMENT: PLAYROOM **- outlet exposed near play kitchen and another outlet exposed by the fireplace ** BATHROOM **- no hazards ** OUTSIDE **- side yard, no hazards, not able to inspect thoroughly due to snow.
Educator has student attendance sheets. The children’s attendance is tracked daily. However, the time of arrival and departure are not noted anywhere. Educator also cannot locate the attendance for September-December of 2025.

Educator has conducted a fire drill every month. However, the date of the drill is not documented, and neither is the time it took to complete the drill.

Tested the smoke detector in the basement space and it worked properly. However, Educator did not have a carbon monoxide detector. All of the required items were observed in the first aid kit.

Licensor inspected all of the licensed, indoor space during the monitoring visit, ,as well as the outdoor playspace. The only hazard that was observed was in the playroom. The brick hearth around the fireplace did not have padding.

Indoor Hazards
Detector in the licensed space was not working in the fall and educator threw it away and has not replaced it. There is currently no carbon monoxide detector in the licensed space.
The water temperature was 135F degrees. Regulations state that the maximum allowed temperature is 120F degrees.
There was a gate that was set aside in the basement space. However, it was not set up at the bottom of the stairs in the playroom during the renewal visit.
There were 2 outlets not covered in the playroom, and 1 outlet not covered in the bathroom.
Licensor inspected all of the licensed indoor space during the renewal visit. The following hazards were observed: ** PLAYROOM - brick hearth needs to be made inaccessible or padded ** BATHROOM - trash was not covered.
Educator does not have progress reports completed for the children.
CPR/First Aid - R Apr 2025
Progress Reports
CPR and First Aid Expired 10/2021. Educator stated she already completed the in person portion of the class on 12/8/21, but has not completed the online portion. Discussed that this must be completed within 1 week, by 12/17/21.
Educator has completed the fire drill for January, but not for February. Discussed that a fire drill needs to be completed every month.

Educator uses the daily attestation forms as her attendance. The time of arrival is noted by the parents as they drop the children off, but the time of departure is not noted. Discussed that this information needs to be added.

Licensor reviewed 2 of the children's records. Both records were missing a signature for the Basic Transport section, and one record also had out of date medical information. Educator also informed Licensor that all of the children are over due for progress reports. Please ensure that these records are brought up to date.
This renewal - enhanced monitoring visit inspection recorded no violations or advisories.

Detector in the licensed space was not working in the fall and educator threw it away and has not replaced it. There is currently no smoke detector in the licensed space.
