A staff records review conducted by EEC at the time of the visit reflected the following: a) Two records did not contain evidence of orientation, interview notes, resume nor reference checks. b) One record did not contain a current health physical. c) One record did not contain evidence of school age work verification. d) Multiple records did not contain ongoing bi-monthly observations nor annual evaluations.
