A developmental checklist reviewed was last completed in March 2022. Other checklists reviewed were completed in May 2024. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).
Licensed capacity for 8 children, two of whom must be school aged. Current enrollment is 9 children. Discussed the educator being over her licensed daily capacity. The educator has had two previous investigations, one in 2022 and one in 2023 where she was found to be over her licensed capacity. The licensor will be speaking with FCC Supervisor Dennis Faulkner regarding the educator attending a compliance meeting in the Worcester EEC Office.
The educator was found to be over her licensed capacity by caring for 9 children. The children are 16-month-old twins, 2 years (2 children), 3 years, 4 years, and 5 years (3 children) old. When the licensor arrived, a mother and another child were approaching the home. The educator asked the licensor to go inside while she spoke to the parent. The educator claimed the parent and child were there for an interview, that she forgot they were coming, and that she asked them to leave. The educator called the parent of one of the children inside the home to pick up their child which they did.

Educator has a notebook with the daily arrival and departure times of the children. Attendance for the day was not accurate with no times recorded and children missing. When reviewing past days attendance, the licensor noted several days when the educator was over her licensed capacity.
Random review of six children's files. One child had an annual update last signed in 2021. Other forms reviewed were signed in April or May of 2024. Physical/immunization documentation appeared up to date.
Educator has a first aid kit in a plastic bin in the bathroom and a mobile kit as well. Kit in the bathroom appears complete with a CPR mouth guard, gauze pads, Band-aids, etc. Educator has a page on the desk with a list of parents first names and phone numbers, but no children's names or information. Emergency contact pages for the children must be included in the first aid kit. Discussed the educator photocopying the Emergency Card page from the child enrollment packet and placing in the kit. These pages must be updated annually.
Reviewed 16 hours of food program certificates. Professional development cannot consist of only food program trainings.10 hours of training are required per year. At least 1/3 of the trainings must include diverse learners. Educator must submit at least 14 hours of training that is not food program related.
Observed various hazards outside such as tools, bug sprays, sunscreen spray, lighters, etc. Charcoal grill was uncovered. Tools and various clutter observed on the kitchen table. Front dryer door in the bathroom was open with clothes strewn on the floor.

CPR and First aid certifications are expired. The educator must register for an in-person course and email the licensor written confirmation by the end of the day today, October 11, 2023.

The educator was caring for 6 children aged 1 year, 2 years, 3 years (2 children) and 4 years (2 children) old. The educator's adult daughter was visiting with her 6-month-old twins. Discussed her daughter not being able to be on premises during childcare hours without a complete EEC background check, including fingerprinting.

CPR/First Aid - R Apr 2025
CPR was renewed on 6/23/21. The educator submitted her CPR twice in the LEAD application and did not submit a copy of her First Aid card. The educator must email the licensor a current copy of the First aid card/certificate as proof of completion.
Pellet stove in the living room has a gate surrounding the hearth, however, the gate is not secured to the wall or the floor. Heaters and stoves in approved space or common space used by children must be surrounded by a fireproof wall or enclosed by partitions, screens, or guards or other similar barricades that are at least three feet in height and installed at least three feet from the heaters and stoves. If non-combustible and non-heat retaining materials are used, barricades may be placed two feet away from the stove. Heaters and stoves that are not used for heating purposes during child care, or are used before child care and are sufficiently cooled to prevent a child from being burned, may be barricaded less than two feet away. The educator must email the licensor photographs of the corrective action taken with the stove showing the distance and different angles of the secured gate.

Observed extension cord and tools accessible on the front porch. Grill tool and charcoal chimney accessible in the play area. Standing water observed in a large brown plastic garbage bin.
