Licensor inspected all of the licensed, indoor space during the visit, as well as the outdoor playspace (fenced in back yard). The following hazards were observed: ** KITCHEN/DININGROOM **- the top drawer in the hutch has a child safety latch that isn’t working and there are choking hazards inside, the cabinets under the sink were not locked and there are bottles of cleaners inside, and the drawer next to the fridge has a child safety lock that is not effective and there are batteries, scissors, and other hazards inside. ** DEN **- top right drawer on the desk has thumb tacks and other choking hazards inside ** PLAYROOM **- no hazards ** LIVINGROOM **- no hazards ** BATHROOM **- no hazards ** OUTSIDE **- no hazards.

Licensor randomly reviewed 4 of the children's records. Of these, 3 were complete and up to date, while the 4th had out of date lead testing.
There is a child enrolled in the program who is diagnosed with [REDACTED] and is prescribed albuterol but the child doesn’t keep an [REDACTED] at the program. This information is not posted in the program. Educator stated the child only needs the medication when they are [REDACTED] Discussed getting something in writing from the doctor specifying what the child needs when they are in the program.
Licensor inspected all of the licensed, indoor space, as well as the outdoor playspace (fenced in back yard) during the monitoring visit. The only hazard that was observed was in the kitchen/diningroom. There were 2 drawers in between the fridge and stove that both have hazards inside and the child proof tabs aren’t working properly.
This follow-up visit inspection recorded no violations or advisories.

Licensor inspected all of the licensed, indoor space during the renewal visit. The only hazard that was observed was in the livingroom. In the top right drawer in the tv stand bureau, there were batteries inside.
There is a deck in the back of the house. There is peeling/chipping paint on the deck and on the lattice under the deck.

The following hazards were observed: ** KITCHEN/DININGROOM - drawer to the left of the fridge has a broken child safety tab and there are hazards inside, drawer next to that has sharp knives and a child safety tab that isn’t working, cabinet under the sink has cleaners inside ** DEN - no hazards ** BEDROOM - no hazards ** PLAYROOM - no hazards ** LIVINGROOM - no hazards ** BATHROOM - cleaners in the cabinet under the sink ** OUTSIDE - no hazards

Licensor inspected all of the indoor, licensed space during the monitoring visit, as well as the outdoor playspace. The only hazard that was observed was in the kitchen. There were 2 drawers that have sharp knives in them and the child safety latches on the drawers were not working.
This renewal - enhanced monitoring visit inspection recorded no violations or advisories.

Licensor reviewed 5 of the children's records. Of these, 4 were complete and up to date. The 5th record had lead testing that was out of date.
Of the 4 children's records that Licensor reviewed, 3 had up to date progress reports, but the 4th was overdue for a progress report.
Educator did not have an Evacuation log for 2024 and stated she doesn’t think she has done any fire drills this year.
All of the required items were observed in the first aid kit. However, the instant cold pack needed to be replaced.
