At the time of dismissal, the staff allowed three children at the other building to walk by themselves down the back hallways, outside, across two crosswalks and into their building. There were school personnel outside helping the children cross the busy traffic of buses but the program staff did not supervise the children arrive to their destination.

The Building and Fire Inspection was not current. It expired on 9/1/2024.
Two staff records were reviewed and found: -One staff record did not have EEC approved CPR and First Aid certification. -Two staff records were missing one bi-monthly observation in May 2024. -Two staff records were missing the staff signature on the Orientation Checklist.
Emergency Medication was reviewed and found one inhaler had expired on 9/2022.

Site Coordinator stated one of the staff called out so they were down one staff and out of ratio. The Site Coordinator called the Executive Director immediately and the Executive Director arrived in about ten minutes to work in ratio.

Two fence gates on the right side of the outdoor playground and play space were open and gave children access to the side driveway of the school where school staff were observed to drive to exit the school grounds.
It was observed that evacuation procedures were missing from two of the four exits in the cafeteria.
Children were observed to begin eating snack without washing their hands.

Written emergency evacuation procedures were not posted in the gym.
A review of two children's records indicated that two Individual Health Care Plans (IHCP) were incomplete. The plans did not include which staff were trained and will be administering treatment while the children are at the program.
A review of four staff records indicated that one staff record did not include a completed Staff Information Form and evidence of their qualifications.

The following was observed to be accessible to children: -In the boys' bathroom across from the gym, rust was visible at the bottom of the metal partitions.
