Outdoor Playspace - Hazards
The provider put up a dog kennel fence with square footage of 100SF which is not enough space for capacity of 6 children. A minimum of 450 square feet is required. There were stones that were a choke hazard accessible in the designated outdoor/backyard area.

Children’s hands were not washed prior to snack.
There were toiletries such as deodorant and soaps in a drawer and on the side of the tub in the bathroom within reach.
The tops of the metal poles around the fenced area were sharp and needed to be covered.
