The following were observed: -A: There were two water stains on the ceiling tiles. Two step stools were soiled. -B: One step in the loft was dusty. Another step had ripped/ peeled white tape. The play kitchen was tippable.
During the visit, an educator diapered a child. She then placed the soiled diaper in the hand washing sink beside the changing table.
