Floors were in need of replacement in the play areas. There was a staff purse within reach in the play area. The diaper cubbie was not secured. The sharp coils were accessible on the heat vents in the rooms. There was a large vacuum not secured in the playroom. The lock on the doors to the boiler room was broken.

Equipment and Materials
There were items such as brooms, dust pans, toilet brushes and other items accessible around the toilet in the bathroom. The bathroom area was in need of cleaning.
The port a crib was dirty and in need of replacement. The baby swing seat cover was dirty and in need of cleaning. The heat vents were in need of cleaning. The port a crib mattress was not adequately secured in.
The Educator had proof of 10.5 hours of professional development training hours. She did not have proof on the premises of the other required hours. Please document all training hours in the PQ Registry.

Progress reports were reviewed for the 16 enrolled children. Seven did not have proof of a current progress report on file for licensor to review.

There was some clutter on the stairs leading to the second floor exit. Discussed that the stairs must be kept clear.

The toothbrushes were store in divided cups, however some of the toothbrushes were touching each other.
