The following were observed: -dusty overhead vents in the bathrooms. -One vent in the bathroom was not fully attached to the ceiling. -A cabinet in the bathroom and in the second room at the end of the building was tippable. -There was a cord over six inches accessible to children in one of the younger rooms. -In the infant room, a shelf was tippable.

During the visit, children were observed using the outdoor "garden" at the end of the building. This area is not fenced and borders the parking lot. This area was not officially approved for use. In addition, at the back of the area, there are a few loose pieces of wood and random discarded items. In addition, there is a large slide structure in a room that was used over the summer. This structure is rated for residential use only. This may not be used in childcare.

The following were observed: -Dragon Flies: There was peeling packaging tape. The sticky side was exposed and cannot be effectively cleaned. A bottle of hand sanitizer, marked keep out of reach of children, was accessible to children. -Butterflies: There were ceramic mugs sitting on the handwashing sink. In addition, there was a fork. -Fireflies: two cords over six inches were accessible to children. The tall doll house was tippable. -Owl and SA room: A tiered bookshelf, two shelves, and a cubby were tippable. A staff cabinet was unlocked, and the following were accessible to children: Vitamin C gummies, Motrin, cleaner. -In the bathroom, there was exposed and chipped plaster accessible behind the changing table slat and on the wall above the sink. A potty seat was sitting on top of a bag and touching hanging clothing items. The contents of a cabinet containing cleaning supplies, and a plunger was accessible to children.

Plastic bags were in an unlocked cabinet under the sink in the Preschool/PreK classroom.
It was observed that the humidifier in the hallway leading to the playground had an unsecured electrical cord. This was accessible to children and a potential choking hazard.

The following was observed and accessible to children: TODDLER BATHROOM - On the changing table pad, there was an open box of gloves and plastic trash bags. In the small cabinet, there was a bottle of cleaning solution and a toilet plunger accessible to children.

A review of two Individual Health Care Plans (IHCP) indicated that both did not include information on which staff are trained to administer the medication and who were they trained by.

Staff meeting documentation
Fall zones adequate depth of impact absorbing material
Staff meeting documentation did not include the length of the meeting. Meetings were also not conducted monthly.
On the playground the slide did not have an adequate amount of impact absorbing material.
One playhouse was broken. A jagged ridge was exposed. Several sensory tables and other equipment were on their side. These were accessible to children and tippable. There were thorny vines accessible to children. The trashcan was broken in several spots, leaving several jagged holes.
