Licensor randomly reviewed 3 children’s files and found all enrollment paperwork to be up to date. Child A had an expired physical which Provider was aware of and working on acquiring. Please acquire from parent as soon as possible.

Fire pit in the backyard space has large sticks inside and was not covered. Observed multiple piles of dog feces on the grass in the yard. Trampoline on the other side of the backyard from play area is not used by the childcare children and for the educator's children. Trampoline has high netting surrounding it.
Small bookcase in the playroom is not secured to the wall or the floor and poses a tipping hazard.

First aid kit is missing a CPR mouth guard or shield. Emergency card pages for the children are included in the kit and signed recently by the parents.

First aid kit is missing a CPR mouthguard.
The home will be heated using baseboard heat. Gas stove will not be used and is unplugged. Educator has ordered an extended gate to surround the gas stove, but it has not come in yet. Educator must email licensor a photo of the gate when in place.
Hot water temperature was 128 degrees. Temperature to the bathroom sink where the children wash their hands must not exceed 120 degrees.
Trampoline has high netting and will not be used by daycare children. Stool to get into the trampoline was in place and must be removed.
