An educator in the toddler/preschool room was observed repeatedly telling a child "All done" when they engaged in an unwanted behavior rather than using positive redirection and clear language.
The rubber baseboard on the cabinet in Preschool 1 is falling off.
The play kitchen set in the Toddler A classroom was not secured.

One staff file was missing the required proof of two reference checks.
An educator in the Toddler/Preschool classroom was observed physically moving children to sit down, repeating the phrases "All done" and "Belly to the table" while pushing their chairs very close to the table. They took away a silicone muffin cup from a child who displayed sensory seeking behaviors which led to the child engaging in other unwanted behaviors such as pulling another child's hair and throwing food.
An educator in the Toddler/Preschool classroom was observed threatening to take a child's food away because they kept lifting their feet off the floor while eating. The educator was then observed doing so, but did give the food back when the child cried for it.
Through a review of four IHCPs the following was found: 2 IHCPs were missing the required training information, and one was missing the symptoms of the chronic health condition.
The table was not cleaned and sanitized prior to children coming to the table for lunch in the Toddler/Preschool classroom.
A students emergency medication was expired.
The walls in the Preschool 1 had chipped paint in several areas.

It was observed that the attendance was not accurate in the preschool classroom. There were 11 children and only 9 signed in.
Rust was found on the preschool playground climber stairs in multiple spots. On the toddler program mulch was found did not have adequate depth of impact-absorbing material.
On an IHCP it was found that the "Who has been trained and will be administering this treatment while the child is at the program" was blank.
Disinfecting wipes were found on the counter and Diaper Cream was found on the bottom shelf of the changing table in reach of the children in the Toddler/Preschool room.

Playground free from hazards; fenced if applicable
Fall zones adequate depth of impact absorbing material
The approved impact absorbing fall zone material(playground mulch) did not meet the 9 inch requirements around/under the structures. The [REDACTED] was able to show the Licensing Specialist that the playground mulch had a depth of 6 inches and then it became matted.
On the day of the visit, there was a visible uneven ground level between where the concrete sidewalk ended and the the mulch begin. This gap was large enough to pose a tripping hazard to children.

The transition in the preschool classroom to lunchtime was not smooth as children were seen waiting for a long period of time at the tables in front of their lunch boxes. The children were instructed not to eat lunch while sitting there for approximately 10 minutes. During this time one Educator was changing the diapers/pull-ups of multiple children individually and the second Educator was trying to read a story to the children sitting at the table, however, many children were hungry and complaining that they wanted to eat their lunch but were not allowed to do so.

There was not a sufficient amount and variety of gross motor equipment/materials available for a grouping of 19 children during the time they were using the indoor gross motor room.
The Preschool Educators were seen walking their grouping of 19 children through the bathroom area when bringing them to the indoor gross motor classroom instead of having the grouping use the hallway route out of their classroom. The Educators did not exercise good judgment nor model appropriate health practices when doing so.

The Educators in the Preschool classroom did not provide adequate supervision of the children in this classroom. Both Educators were engaged in custodial tasks (cleaning/sanitizing rest mats, putting away the children's rest materials and delivering snacks to children at multiple tables) which did not allow the Educators to be in close proximity to children who were seen hitting each other, or taking food from each other's plate during snack time.
The program was not being soundly administered on the day of the visit, as a [REDACTED] child [REDACTED] at the program was allowed to sit in the kitchen/staff area of the program while [REDACTED] parent was in the preschool child/staff ratio. The current license only admits children up to the age 9 to be cared for at the program. The [REDACTED] did not use good judgment when [REDACTED]agreed to allow this [REDACTED] pick up [REDACTED] child from school and bring [REDACTED]back to work with her on the day of the visit.
Although there are two sinks in the toddler classroom, teachers and children were seen washing their hands after diaper changes in both sinks. An Educator was also observed using the counters around both sinks to prepare the afternoon snack of pretzels and applesauce. This Educator was seen plating the food and lining the plates/cups around the designated diapering/toileting sink as well as the designated food preparation sink. One child was observed grabbing pretzels and eating them at the same time he was washing his hands after a diaper change at the designated diapering/toileting sink.

Through a review of [REDACTED] staff files for staff who have been employed for multiple years, there was no evidence of a current year's completed annual performance evaluation.
At the time of the visit, the emergency/life saving medication was not listed on the allergy posting
The paper attendance records in three (toddler B, preschool one, toddler/preschool classroom) did not reflect the actual number of children present in each of these classrooms.
The afternoon snack was all teacher led, and did not allow the children an opportunity to foster their independence and self help skills. Also, children were not provided the opportunity to help clean up their rest materials, or mats.
Through an inspection of the storage of food in the kitchen area, opened packages of rice cakes and pretzels were not being stored in sealed or closed containers.
Although the Building Inspection was current, the Fire Inspection had three violations on it that needed to be corrected.

At the time of the visit, peeling plaster was observed on a part of the the toddler classroom wall.
At the time of the visit the following was noted regarding infection control/hand washing practices: 1) Child A was not instructed to immediately wash his hands after having his diaper changed. Child A was allowed to sit at a table, and look at a book while the Educator completed the task of cleaning and disinfecting the changing table, before she asked this child to wash his hands. 2) The table in the toddler classroom was not cleaned and sanitized before children sat at it and ate afternoon snack. 3) The children in the toddler and preschool classrooms were not instructed to wash their hands prior to eating snack 4) A preschool child got up from rest, went to the bathroom area, and pulled off her own pull up and then went to the snack table and ate snack without washing her hands
