The First Step and Toddler classrooms lacked a variety of materials such as a cozy area, art supplies, dramatic play, and science/sensory.

A review of the infant attendance showed seven children were signed in with six children present. Two of the students signed in were in a different classroom and one of the children present was not on the attendance. A review of the First Steps classroom showed only two children were signed in while 5 were present.

Physical care of children
A review of staff files showed that three educators have not completed the Essentials 2.0 trainings.
Through a review of five children's files, three had expired emergency contact and release forms, transportation plans, and developmental history. Two of the files had expired physicals.
One staff member did not have a staff file, two staff files were missing proof of qualifications.
The First Step and Toddler rooms were lacking art, science and sensory materials. The Toddler classroom had the dramatic play materials stored out of reach.
An educator was observed picking a toddler up by their arm to move them out of the way.
The program did not have an adequate depth of impact absorbing material around the climbers.

During an observation of diaper changes, it was noted an educator did not wash their hands between diaper changes.

Toxic Substances
A review of five staff files showed the following was missing: Three of the staff files was missing documentation of the date of interview. Four of the staff files was missing proof of two references.
A review of the children's record checklist showed the following: 4 children had expired physicals 3 children were missing proof of lead test
A review of the staff records checklist showed that four staff did not have evidence of First Aid Certification and four staff had expired First Aid certification.
The infant classroom had four children on their attendance, while there were five children present.
The required training information and parent signature was missing from one student's IHCP.
The following was observed in the infant classroom: Diaper cream, room deodorizer, and adult scissors were observed stored in an unlocked cabinet and within reach of children.
Three pieces of climbing equipment were observed on the playground without a fall zone or impact absorbing materials.

Several pieces of equipment were not secured.
* A light cover was observed not securely attached to the ceiling posing a risk to children playing underneath it. The program director promptly removed it.
The outdoor play area does not have the required depth of sand in the fall zone areas.
* Air freshener was observed stored within reach of children in the gross motor room. * A toddler was observed using a small counting bear. This is considered a choking hazard.

During the visit some furniture/equipment was found to be tippable posing a potential hazard.
During the visit Licensor observed toddler children walking out of the bathroom. Educators were in the bathroom sitting changing other children's diapers. Educators were not positioned to adequately supervise all children in their care.

During the visit Licensor observed a bottle in one playpen and a sippy cup in another playpen. When first bottle was seen Director was asked if an infant was drinking it while in the playpen and Licensor was told no they had just put it down there while they were busy doing something else. When sippy cup was seen in 2nd playpen Director was asked a second time if an infant was drinking it in the playpen and Director stated the infant was.
Licensor observed 4 infants being put in their highchairs to eat and staff did not ensure their hands were washed.

Licensor observed no bottles of cleaner or disinfectant at the diaper changing station utilized by toddler and preschool classrooms.

When Licensor arrived at the program the Director answered the door. After speaking for a few moments the Licensor asked if the Director was in ratio and she stated "yes, in the infant room". When Licensor opened the infant classroom door there were 4 infants and no staff present. The assistant teacher had also left the classroom.
Licensor observed an infant in a baby rocker chair drinking their bottle. Infants must be held while fed a bottle.
Overall attendance was not accurate. Program has an arrival and departure attendance at front door and classrooms use check off attendance. Timed attendance had 2 infants, 7 toddlers and 9 preschoolers marked in. There were 4 infants, 8 toddlers and 9 preschoolers in attendance. Neither the infant or toddler classroom had completed their check off attendance rosters.
Interactions
Teacher stated they only used disinfecting wipes on tables prior to snack and did not use a 2-step cleaning and disinfecting process.
Licensor observed potentially hazardous items (ie: staff purses, disinfecting wipes and multiple cans of air freshener) within reach of children in classroom and gross motor space.
Licensor observed, and overheard when not in the classroom, the preschool teacher yelling at the children repeatedly. The teacher was heard yelling things such as; "Get Up" and "Get back over there and sit down". The teachers interactions were not nurturing and responsive and she did not provide guidance to children in a positive and consistent manner.

At the time of the visit the staff schedule was not posted in an area easily visible to all parents, educators and visitors. There was a staff schedule posted in the PS classroom however it was not current as required.
