Space (indoor / outdoor hazards) - R Apr 2025
Licensor walked through all licensing space and found the following hazards: *KITCHEN: Gas stove requires covers on knobs. Educator said she may be able to shut it off during program hours. Please respond with how you plan to make gas stove safe for children to be around.
First Aid Kit
A small little tykes' climber that is over 18 inches in height was observed without the required fall zone material. Discussed removing it from the play space.
Ongoing Professional Development
Space (indoor / outdoor hazards) - R Apr 2025
First aid kit appears complete with a CPR mouthguard, Band-aids, roller bandage, gauze, gloves, an instant cold pack, etc. Educator uses a touchless thermometer. Discussed the kit being mobile and going outside with the educator and the children in a bag or backpack. Emergency card pages for the children were out of date. Discussed parents re-signing and re-dating the bottom of the pages if no information has changed.
The educator has only completed 2 hours of the required 30 hours of professional development training for this licensing cycle. The educator must complete and maintain proof (certificates) of 10 hours of professional development training per calendar year. At least one third of the required professional development must address diverse learners. The educator will need to email the licensor proof of the completed 30 hours of training.

Progress Reports
Progress reports are not being completed as required. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older). A copy of the report must be given to the parents and another copy kept in the children's files.
Observed a large pile of sticks uncovered and accessible by one of the sheds outdoors. One of sheds was unlocked and contains old paint cans, a propane tank, and other possible hazards.
This monitoring visit inspection recorded no violations or advisories.
Outdoor Playspace - Hazards/Porches and Decks
One large shed needs a padlock to prevent children access to the hazards inside. Backyard play space. Educator lives on a quiet side street. Little Tykes slides under 18 inches in height. Plastic child sized picnic table and basketball hoops. Above ground pool with a locked ladder that is up. Educator does not use the pool with the children.

Waterproof changing mat. Educator does not have a disposable layer between the children and mat.
