At the time of the visit, the preschool classroom went on a walk and left one child's emergency medicine at the center.
At the time of the visit, the attendance indicated that there were 10 children present; however, there were 12 children present. Two children's time of arrival had not been noted; therefore attendance was inaccurate.

At the time of the visit, three children's files were reviewed and the following was found: one file was missing documentation of date of admission, one file was missing a [REDACTED], one file was missing documentation of [REDACTED], and three files were missing offsite consent form.
