A sampling of children's files was reviewed. One child's file included a Transportation Plan that was signed/dated by the parent over a year ago and is now expired.
Individual packages of Graham Crackers were not stored in the original box. These packages did not include an expiration date. Therefore, EEC could not determine if the food had a current expiration date.

In the Infant 2 classroom, a large play mat and other toys were located in front of the exit door, blocking the egress to the outside. Exits and evacuation routes must be kept clear of obstructions.
One child had documentation from the doctor that the dosage for their emergency medication for a mild allergic reaction was 5 ML and also noted was the dosage of 6 ML. The dosage amount of medication within the paperwork needs to be consistent so that the program knows which dosage to administer if needed.
The program must provide refrigeration and storage for food at not less than 32F, nor more than 40F for all food requiring refrigeration. 1) There was no thermometer located in the kitchen refrigerator; therefore, the temperature could not be determined. 2) The thermometer in the Toddler 2 refrigerator read 46F.
The program did not have a current Health Permit. Please email EEC when the current permit has been uploaded to the document library.

A sampling of staff files was reviewed. One staff file did not include a completed Verification of Experience Form to qualify the staff member for their assigned position.

One staff file reviewed did not include a current physical exam.
In the MMZ play area, there was a rug that had 2 corners that were curled up which caused a potential tripping hazard.
1) One child's Individual Health Care Plan was signed/dated over one year ago. 2) The Plan did not include staff who were trained in administration of the medication and who trained them to administer the medication.
One child's Emergency Medication was current. However, the prescription label had expired 12/22.
A large tub of Cream Cheese stored in the kitchen did not include an expiration date,
One child's medication form included a different emergency medication dosage amount from what was prescribed by the child's doctor.
One child's Individual Health Care Plan noted that the child had a second emergency medication for mild allergic reaction. the parent has not provided this medication to the program.

Three children's files were reviewed. A Medication Consent form was signed and dated by the parent; however, the form was not complete. The Individual Health Care (IHC) Plan did not include the potential consequences if the medication is not administered and side effects of medication signed/date by the child's doctor.
In the Preschool classroom, written emergency/evacuation procedures were not posted next to one exit.
One child's enrollment forms included tht they have an allergy to a medication. This was not noted on the Allergy/Medical Condition List.
The practice evacuation drill log for the past year was reviewed. The latest drill noted was 1:30PM. The program closes at 5:30PM. The program needs to ensure that drills are being conducted during all hours of operation.
