Current consent for administered medications
1) One child's Medication Consent Form had a different medication dosage from what was noted on the IHC Plan. 2) One child had a medication prescribed for a specific allergen. The parent had not completed a Medication Consent form for this medication to be administered to the child, as needed.

EEC observed a child's pull-up diaper being changed while the child was standing on the floor of the bathroom. Even when standing up, a child should be changed on a diaper changing surface that must be covered by a disposable cover.

In the on-site outdoor playground, there was a small wooden slide structure that had sections where materials were peeling.

In the Infant classroom, an educator changed a child's diaper. The educator washed their hands after diapering; however, the child's hands were not washed after diapering.
In the preschool classroom, there were 2 chairs and a bag in front of the stairs at the exit/evacuation door to the outside. The program must ensure that exits and evacuation routes must be kept clear of obstructions.

The exit door leading outside from the preschool classroom did not include a written evacuation plan.
The Allergy/Medical Condition List did not include that a child was allergic to fragrances and synthetic materials.
The Fire Drill log was reviewed. The program has not been documenting consistently the routes used so that primary and alternative exits are being practiced throughout the year,
In the infant classroom, the program was using the handwashing sink to fill up water containers for children's bottles to warm up. Facilities used for hand washing after diapering or toileting must be separate from facilities and areas used for food preparation and food service.

Documentation including 2023 practice evacation drills did not include the route used for each evacuation. The program must practive alternative routes.
There was a large amount of sand (from the sand box) on the pavement area of the playground as well as the poured rubber area of the playground. The sand made these surfaces slippery and a potential hazard for children.

1) One child's enrollment forms were signed/dated by the parent over one year ago. 2) One child's file did not include a current progress report.
On the day of the visit, EEC observed several children playing on the far end of the on-site playground in and around the play structure. There were no staff members in that area of the playground supervising these children.
The program's license includes a group of 9 toddlers. This classroom is being operated as an Infant/Toddler classroom of 9 children. The program did not ensure that the current license reflected the new mixed grouping prior to changing the grouping.
In the Infant/Toddler Classroom, there was a tall wooden shelving unit that was tippable.
1) At the on-site playground, there was a hose that was stored on the ground; storage in this manner was a potential tripping hazard for children. 2) At the on-site playground, EEC observed a child playing on top of the sand box cover and another child playing in the sand with the sand box cover still covering the majority of the sandbox. When children are in the sandbox, the cover should be removed and stored inaccessible to the children.
