A sampling of medications were reviewed and the following was found: 1. Two children’s medications were expired. 2. The affixed label on the box of a medication was expired.
A toddler/preschool classroom went outside without a child’s emergency medication resulting in the medication not being readily available.

A sampling of staff files were reviewed and the following was found: One file was missing evidence of two reference checks.
A sampling of children’s records were reviewed and the following was found: One file was missing a Medical Consent form.
In a preschool classroom, one child’s arrival time was not documented on the attendance. A preschool classroom was out of ratio when a teacher left the grouping.
One preschool classroom did not have evidence of an emergency evacuation drill occurring in January of 2020.
