She keeps a personnel file on assistants. However, two assistants were missing the physical exam.
Random review of children’s records. Child 1 was missing parent signature on general permission form. Child 2 and Child 3 were missing LEAD test.

Licensor observed, the following indoor hazards accessible to the children: 1) four sharp knives that were stored inside two unlocked drawers in kitchen area. Licensor observed the following outdoor hazards accessible to the children: 1) a loose black hose that was on the pathway to the outdoor play space, that presented a tripping hazard for the children. 2) an uncovered gas tank that was attached to the grill on the deck area.
