Licensor observed that the program's evacuation log was missing the exit route, floor level, meeting place and smoke detector log to document program's monthly test of smoke detectors.

During the visit, a snack was served to the children by the educator. The children's hands were not washed before and after the snack with liquid soap and running water, using friction, in accordance with the Department of Public Health guidelines.

After the review of five children’s files who were present during the visit, Licensor observed the following : 1) two of the children were missing up to date progress reports.
Licensor observed that the PQR# 9696907 expired on 7/27/19.
After the review of the five children’s records who were present during the visit, Licensor observed the following : 1) Three of the children were missing the child’s physical description in the enrollment packet. 2) one child was missing the physician’s health care information in the enrollment packet. 3) Three of the children were missing up to date physical examinations. 4) Two of the children were missing {REDACTED} results. 5) Three of the children were missing annual update.
Licensor observed the First Aid kit was missing one child’s Emergency Card information.
Licensor did not observe the following after reviewing the dogs' records: 1) one dog was missing up to date vaccination. 2) Licensor did not observe evidence of one of the dog’s vaccinations.
