The licensor reviewed the first aid kit and observed the following: 1)Four different children had expired emergency cards.
The licensor reviewed the parent handbook and observed that the educator was missing information pertaining to assistants information, license capacity, and EEC information.
The licensor reviewed a sample packet of children's records and observed the following: 1)Child #5 has a expired physical in child file. 2) Child #8 has an expired lead test in child file. 3) Child #9 has an expired lead test in child file.
At the time of the visit, the licensor observed a toddler sleeping infront of the secondary exit.

Licensor observed that the playroom wall was not in good repair. There was a hole in drywall, the plaster was cracking.
Licensor observed that the program was not using a playpen as recommended by the manufacturer. She added an extra mattress on top of the original playpen mattress.
