Two children with medication on the premises did not have a Medication Consent Form signed by a parent.
One child's Individual Health Care Plan included the administration of Benadryl. This medication was not on the premises, therefore it was not immediately available if needed.

Plastic shopping bags were located in an unlocked kitchen cabinet and accessible to the children. In the children's bathroom, plastic bags of pulls ups and pouches of diaper wipes were accessible to the children.
During the visit, three staff files were reviewed. One file did not include the verification of two references. Two files did not include a current physical. One file did not include evidence of immunity to Measles, Mumps, and Rubella. One file did not include evidence of First Aid Certification. Two files did not include documentation of the 12 EEC Essential Trainings.
The following hazards were observed outdoors and were accessible to children: Several bolts on the chain-link fence were protruded inward towards the playground. Two sections of wood, secured at the bottom of the fence were rotted and must be replaced. The paint was peeling on the red wooden car structure.
During the visit, an educator's purse was stored in a cubbie and accessible to the children. Plastic bags were accessible to the children in the bathrooms and kitchen area.
A current Children's Record Checklist was not available for review during the visit.
