Safe Sleep
At the time of the visit, an infant was observed sleeping on a boppy pillow. The child was not picked up and placed in a crib until EEC staff acknowledged that the child was sleeping. This was several minutes after the child had been observed asleep by EEC staff.
At the time of the visit one child reviewed had an expired individual health care plan.

The infant room attendance was not accurate. The attendance sheet showed 11 infants signed in but only 8 infants were present at the time. Program wide attendance sheets did not indicate time-in and time-out.
One staff file only had documentation of a resume.
The Staff Schedule was not current or posted.
One educator was using the bathroom (in the classroom, with the door shut), while another educator was supervising 13 preschool aged children.
A mixed age grouping of 10 toddlers and preschoolers were observed in the preschool classroom.
Chipping paint was observed on the bathroom door in the Toddler/Preschool classroom.
One childs emergency medication was not on site.
