A review of three staff files, Licensor noted the following: 1. One staff was missing evidence of EEC 12 hours strong start modules. 2. Two staff were missing evidence of program orientation.
1. The staff schedule posted was not current. 2. Two staff from another site were working at the program but they were not listed on the staff schedule.
One child was missing the consent form for the prescription medication.

One emergency prescription medication was not in the container in which they were originally dispensed and with their original labels affixed.
A review of six emergency medications found that four children did not have the consent forms for the medications.

At the time of the visit there were seventeen children in the cafeteria with one staff while the other staff went to the second floor on several occasions leaving the program out of ratio.
A review of the fire drill log found that it was only conducted in December 2022, and did not include the exit routes or effectiveness of the drill.
