At the time of the visit, the licensor observed a lady by the name [REDACTED] in the kitchen. [REDACTED] stated sometimes she comes to the child care program to cook for the educator, licensor asked her how often and she stated sometimes. When educator arrived, educator stated that [REDACTED] came to visit her and decided to cook for her because she was not feeling well. Licensor informed the educator this the second time, this year, that she is observing a person who is not listed on the list of household member or regularly on the premise present at the time of the visit.
One child does not have a medical record and one child’s medical record is due for update.

When the licensor arrived, she observed children jumping on the sofa in the kitchen. The children were wondering between the kitchen and the playroom. The educator’s assistant proceeded to tell them to go to the playroom but with little success, the children returned to the kitchen shortly after.
At the time of the visit, the exit route through the kitchen was barricaded by a chair, a laundry basket and other hazards. See picture.
The educator is not documenting the assistant’s attendance.

Some children’s enrollment forms are incomplete, the physical description and other parts are left blank. One child’s medical record is due for update.
The educator stated she conducts the evacuation drill every month. However, per evacuation drill log, educator did not conduct evacuation drill in the following month: April, May, June, September and October. Educator is also not documenting the evacuation drill accurately.
There is no exit sign on the exit route near the playroom.
Licensor observed children’s file stored on top of the diaper changing mat.

The bathroom is on the second level of the house and there is no gate at the top of the stairways in the second level.
