Four records were reviewed at this visit. Two of the children's records were missing the new medical form and the annual update form.
Two day care children have allergies. This information needs to be posted.
The individual plan needs to be renewed for the child with the allergy. If the child requires a medication, the program must have the required medication and a parent medication consent form.

The program was not meeting license capacity. The program was caring for seven children none of whom were of school age. The program must always maintain within the license capacity.
One child did not have a complete enrollment record. The children's enrollment records must be received prior to children's start date.
The CPR expired on 9/5/18. The CPR course must be completed as soon as possible. The educator must provide a copy of the current CPR card once the course is completed. The FA certification is current.
The first aid kit was missing a cold pack.
A fire hydrant was located on the floor accessible to the day care children. The fire hydrant must be placed in an area out of the reach of the children.
