During a review of the staff records checklist, documentation was missing for at least three educators, including staff information forms, BRC suitability letters, and staff orientation checklists.
During a review of children's records, at least two children were missing medical paperwork, including Individual Health Care Plans and medication consent forms.
It was observed that two educators were not in close proximity to children who were playing in a large field during outdoor play. The educators were sitting on the picnic table that is located on the back top, while three children were wrestling in the field and another group of children were playing near the wooded area at the farthest point of the field.
Two children were missing written parental consent to administer emergency prescription medications.

Medication
One file was missing a PQ registry form, proof of MMR and BRC Suitability. Another file had an expired PQ Registration date, and evidence of an annual evaluation. Another file was missing a physical and proof of MMR.
The program had 34 children. The attendance was accurate. The program did not have a sufficient amount of qualified staff for the 34 children attending. The program had one site coordinator and two assistants.
During the visit an educator was observed writing up a document, while children were on the other side of the room grabbing each other, running and sliding across the floor into shelves and bleachers, climbing on top of the closed bleachers and getting into the stacked chairs and mats.
Children did not wash their hands with running water and soap before having snack, they used hand sanitizer.
One child's Albuterol expired on 8/2023.
