Diaper changing was observed in the infant/toddler room, and the children and educator did not wash their hands with running water and soap after each diaper change, and the diaper mat was not washed and disinfected after each use.

While observing the preschool classroom outside the children were playing with woodchips. A child told the Licensor they are not allowed to play with woodchips, in response the Teacher told the children to line up and go inside. While walking around the playground the Licensor observed a child punch another child in the back, when the Licensor pointed it out to the Teacher, they did not address the hurt child or the child who punched them. The Teacher in the classroom used the phrase "No thank you" often when they wanted a child to stop something then gave the children no guidance.

While observing the preschool classroom of 17 children on the playground, The Teacher lined the children up at the fence. One child did not get in line, the Assistant Teacher was trying to get that child to go inside with the group. The Teacher took the lined up children and went inside, leaving the Assistant Teacher alone with one child on the playground.
Sufficient materials & equipment visible, available, and accessible
A classroom of 17 children were all made to sit at the tables for a project that involved one pan with marbles, the children had to wait their turn to get the pan. Children were not allowed to play with toys until they did the project. Children were leaving the tables and told to sit back down. Children were getting restless and frustrated waiting their turn.
In the back right preschool playground a large route was exposed causing a potential entrapment and tripping hazard.

Indoor Space
The rug in preschool classroom 1 was frayed and a potential tripping hazard. Preschool room 1 located on the second floor did not have window guards. The window guards in the new preschool room have entrapment areas. The carpet in the new preschool room is a tripping hazard. The book area in the new preschool room is located in a high traffic area.
Educator’s were not aware of the symptoms that require Albuteral for a child with Asthma. The child did not have an Individualized Healthcare Plan signed by the doctor.
This monitoring outside differential licensing cycle visit inspection recorded no violations or advisories.
Children at the tables became upset when they were asked to wait too long to start an activity. Some children cried, some children started playing around in their chairs, some children began bothering other children at the table. Children got up to play with toys and the educators told them no and to sit back down, until they completed the planned activity. Children’s individual needs were not being met.
A child was asked to take their hands out of their mouth, but was not encouraged to wash their hands after removing them.
