The table children sat at for lunch in the toddler room was not cleaned and sanitized prior to children sitting for lunch.
The toddler playground had rubber mulch, the lining under the rubber mulch was exposed throughout the playground, resulting in a tripping hazard.
The Building Inspection Certificate and Fire Inspection were expired.
There was chipping pant on the window sill in the toddler room. And chipping paint in the bathroom.

The Toddler classroom had limited materials and interest areas. The space had limited areas for small and large group activities.
The educator preparing lunches washed hands then picked up the trash and moved it, then put gloves on and began preparing children’s food without washing their hands first. The food was heated in the microwave for one minute, the temperature was not taken, another meal was microwaved for three minutes and no temperature was taken. The educator wore the same gloves to prepare all children’s individual meals.
The shelf in toddler room was broken and had sharp edges. An outlet cover was missing, walls had handprints, markers, crayon and food on them. The floors and the floor boards had food crumps and stains. The car rug in the toddler room was stained and dirty. The infant room had food on the table and the floor, the diaper changing area had a dirty dust pan on it. The gate between the kitchen and the eating area was stained and covered in paint, food and dirt.

The gate on to the playground was between 3.5 and nine inches which could be a potential head entrapment.
