-A review of three Staff Records during the visit indicated that one staff record did not include evidence of EEC Essentials 2.0 training. -Upon review of the program's Staff Record Checklist that is listed as being updated on 9/9/25, six out of 20 staff did not have completion dates listed for completing the Essentials 2.0 training.
A review of three Staff Records at the time of the visit indicated the following: -One staff record did not contain evidence of bi-monthly observations or a completed BRC. -A second staff record did not contain evidence of bi-monthly observations, completed BRC, current PQR, or EEC Essentials 2.0 training. -A third staff record did not have evidence of a current physical exam and MMR immunity. The program must maintain a current and complete staff record for each staff member, including immunization records.
The program did not keep accurate records of emergency evacuation/fire drill logs with the correct month and year, as they were dated 2023/2024. The Director stated they were using "old" forms that had been pre-printed and the year was incorrect. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.

Current consent for administered medications
The Staff Schedule was not current on the day of the visit (10/22/25) as an employee left the program on 10/10/25. The Staff Schedule must be updated to reflect the staff change as well as the maximum number of children in the class. The Staff Schedule must be updated to ensure accuracy of all staff titles and positions as one educator was listed as an Assistant and by themselves in the early morning in the Rainbow class. When asked about this, the Director stated that the educator has their teacher qualification. -The Staff Schedule must keep the schedule current and posted in an area easily visible to educators, parents and visitors.

Individual Health Care Plan (IHCP)
A review of the program's Children's Record Checklist at the time of the visit was indicated as being updated on 9/9/25. The checklist indicated the following: -Out of 61 children listed on the checklist, 36 children lacked current enrollment forms (Transportation, First Aid and Emergency Medical Release). The program must maintain current records for each child and update annual forms according to EEC regulation 7.04(07).
A review of three Staff Records at the time of the visit indicated the following: -Two staff records did not include evidence of a completed BRC. In a review of the BRC Navigator system, both of the staff members did in fact have a suitable BRC. All Staff Records must have evidence of a completed BRC as is required 7.04(18)(c)(05).
The program did not have a current Building and Fire Inspection for review at the time of the visit.
A review of six Children's Records during the visit indicated the following: -One child with a diagnosed medical condition did not have a current IHCP. -One child with two diagnosed medical conditions did not have an IHCP for either diagnosis. -One child with a diagnosed medical condition did not have a current and complete IHCP. The most recent IHCP for the child was dated in June of 2023.
A review of six Children's Records during the visit indicated the following: -One child with a diagnosed medical condition and prescribed medication did not have a current Medication Consent Form. The Director stated during the visit that the parent verbally told the educator that the child no longer requires medication. However, the program did not have any written documentation from either the parent or the child's pediatrician confirming this. -One child with two diagnosed medical conditions did not have an IHCP for either diagnosis. The Director stated during the visit that the child did not require medication but the record did not include evidence of this from the child's pediatrician or parent.

A review of the BRC Navigator indicated the following: -One staff member is listed as "pending" with a submitted date of 9/10/24. -One staff member's BRC expired on 8/5/24. -One staff member is designated as being suitable, but is listed as "inactive." All staff members must have a currently suitable (or provisionally approved) determination in order to work in the program. Otherwise, these individuals should not be working in the program until such time that this has been completed.

A review of the Staff Schedule submitted to the LEAD document library on 9/25/24 indicates Assistant Teachers are left alone unsupervised in one preschool class and in two toddler classrooms. Assistants must never be left alone. The Staff Schedule also indicates that these classrooms are over-ratio at the end of the day, appearing to leave one educator with the maximum capacity number of children. The Staff Schedule must be revised to reflect the program is maintaining sufficient staffing and ratios at all times.
This investigation inspection recorded no violations or advisories.
The Staff Schedule was not current and posted in the program. During the visit, the Director stated that there has been a recent change in staffing and the program is in the process of updating its schedule.
Upon a review of three Children's Records during the visit, the following was identified: -One child's record did not have evidence of a parent signature on the Emergency Release Form.

Upon a review of five children's Individual Health Care Plans, the following was identified: -One IHCP was not current and expired on 11/2/22. -One IHCP did not have the names of the educators trained in the child's medical condition and the name of the individual who trained those educators.

Upon a review of medications stored at the program for children, one medication (REDACTED) was not stored in its original container. The medication did however, have the affixed prescription label secured on the(REDACTED) itself.

The Staff Schedule was not posted at the time of the visit. The schedule must be posted in the program where all staff and visitors may view it. Upon Licensor's request, administration updated the schedule and uploaded it to the program's LEAD document library by the end of the day on 9/24/24. However, this must be revised as communicated via telephone call and email and outlined in this report.
The Fire Inspection Certificate was not available for review at the time of the visit.
A walk through of the facilities indicated the following: -In the Firefly classroom, to the right of the utility sink, there was an air purifier with a wire longer than six inches accessible to children.
Medication
The Building and Fire Inspection are not current. During the visit, the Director stated that the inspectors visited the program last week and an issue was identified in the church needing to update heat sensors.

Upon a review of nine (9) Individual Health Care Plans during the visit, the following was identified: -One Child's Record had an Individual Health Care Plan (IHCP) that did not include the names of the educators trained in the child's medical condition and the name of who trained those educators.
A review of five staff files indicated the following: -Two did not have documentation of a BRC (Background Record Check) Consent Form.
A review of medication stored at the Program indicated the following: -One child’s prescribed Epi-Pen had expired in March of 2019. -One child’s (REDACTED) had expired in (REDACTED). Therefore, two children at the program did not have access to (REDACTED) in the event of a possible emergency.
