Medication
A review of three staff files indicated: -One file did not have evidence of First Aid training.
A review of three staff files indicated: -One file did not have evidence of a Background Record Check (BRC) Consent form.
A review of three staff files at the Program indicated: -One lacked evidence of a current physical examination.
A review of six children’s records indicated: -One file did not have evidence of a physical examination. -Two files did not have evidence of a current physical examination. -Four files did not have evidence of lead screening. The Program may accept a copy of a physical examination, immunizations, and lead screening from a child’s physician or a written parental statement that this required information is on file with the child’s school.
The Program did not have a Staff Schedule posted. The Program must post a current Staff Schedule in a location that is easily visible to educators, parents and visitors.
The Building Inspection Certificate expired on 9/1/19.
The Program did not have Evacuation procedures posted at each exit as is required for emergency preparedness.

-A child with a (REDACTED) (according to the child's physical examination record) did not have evidence of an Individual Health Care Plan (IHCP) and accompanying Medication Consent Form for a prescribed (REDACTED). Therefore, a child did not have what would be needed during a possible emergency.
A review of medication stored at the program indicated: -Two prescribed medications had expired. All emergency medication must be readily available.
