A child had a coin battery in their hand. Licensor observed it and asked the child for it. The child gave the battery to the licensor. The licensor had to inform the assistant who was in close proximity to the child that the child had the battery in their hand.
The back exit storm door closer was broken. The outdoor area gates were unlocked. One gate allowed access to the remainder of the yard to include a trampoline, deck stairs, a fire place and leaf blower. The other gate was opened to the underground pool, the pool was covered. The pool cover had sitting water on top of it.

The side exit route was being obstructed by an unraveled water hose. The front door exit route was being obstructed by a large work van.
The educator did not have a completed staff record for the assistant staffing the program. The record did not include the assistant's medical form or completed trainings. The assistant's license and CPR/First Aid certificate was also not printed but the assistant was able to show licensor on their phone an electronic copy.

In the outdoor space, the gate to the outside of the fenced in play area was open allowing access to the remainder of the yard with hazards including but not limited to building materials and a trampoline. The gate to the inground pool was open allowing access to the pool.
The educator is not alternating exits on a monthly basis when conducting the evacuation drills.
All of the children's records were reviewed. Four children need updated annual forms, four children need updated physicals.
One of the pack and plays being used for napping materials did not have the original mattress and the other had the original mattress but was broken.

A five month old was observed to fall asleep in a pack and play with a soft mattress and playmat. A piece of plywood was under the original mattress.
In the bathroom under the sink, plastic bags were accessible, hand sanitizer on top of the sink and a trash can with no lid. In the playroom, the toy kitchens were not secured posing a tipping hazard.
The six children present record's were reviewed. Three children did not have their annual update form, three children did not have current physicals, three children did not have their lead testing and one child did not have their vaccines.

All of the children's progress reports were reviewed, four of the children required an updated progress report.
The stairwell leading to the outdoor space was observed to have chipping and peeling paint.
The educator was observed to change a child's diaper and not wash the child's hands after diapering.
In the outdoor area, the two gates of the fenced in area were open to the remainder of the yard with hazards. In the fenced in area, their was broken wood pieces, small rocks which could pose a chocking hazard exposed from the lifted grass, a flipped over basketball hoop and an aluminum tube. The bottom of the stairs did not have a gate.
The second exit has several hoses and wires in the exit path causing an obstruction.
The children's records were reviewed, there were three children's progress reports which were not observed to be completed.

In the bathroom, there was a small plastic trash bag roll and an air freshener accessible to the children. In the outdoor space, there is a slide which measures over 18 inches and does not have a fall zone.
