A walk through of the program's outdoor playground at the time of the visit indicated the following: -There is exposed rotten, splintered wood with a large protruding nail to the left of the gate entrance to the playground that children must pass through when entering and exiting the area. This is a safety issue that needs immediate repair/replacement. -On the playground, the large multi-colored climbing structure has portions of broken plastic that appear to have been previously covered in duct tape which poses as a safety hazard for children. The structure also is covered a thick layer of dirt and debris which requires cleaning. -To the left of the basketball hoops hung on the fence, there are two panel sections of the black wired fence on the bottom that are not secured and is protruding onto the playground, accessible to children. This requires repair. -On the playground there is a round plastic laundry basket that is hanging on the fence that is broken and in need of repair/replacement. Additionally, there is a basketball hoop hanging from the fence that is broken and is also in need of repair/replacement. -To the right of the hanging baskets along the fence there is a wooden shelf with toys. This shelf has a rusty protruding nail that is accessible to children that must be repaired/replaced as it poses as a safety hazard. The outdoor space must be properly maintained and free of hazards.
