Educator has been bringing the children outside and playing outside of the fenced in area in the back yard. Educator lives on a busy street and therefor it is considered hazardous for the children to play outside of the fenced in area. Discussed that this is not allowed, per regulations.

Licensor inspected all of the licensed, indoor space during the visit, as well as the fenced in area in the backyard. The only hazard was in the kitchen/diningroom. The drawer to the right of the sink has sharp knives inside and the child proof tab isn’t working.

CPR/1st Aid Issued 3/17/23 and expired the end of 03/2025. Educator is scheduled to take a new class on 4/29/25. Licensor consulted with Supervisor [REDACTED] on this matter. Educator is aware that she must provide documentation that verifies that she completed the CPR/1st Aid course before she can open her program on Wednesday.

Licensor randomly reviewed 4 of the children's progress reports. Of these, 3 were complete and up to date, but the 4th report was overdue.
Tested the carbon monoxide detector in the living room and it worked properly. Tested the smoke detector in the hallway and it did not work The 1st Aid Kit was missing a CPR mouth guard, and the emergency forms need to be updated.
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: **KITCHEN/DININGROOM** - top drawer left of the dishwasher had sharp, steak knives inside **LIVINGROOM** - no hazards **BEDROOM 2** - no hazards **BATHROOM** - no hazards **BASEMENT: DAYCARE ROOM** - no hazards **OUTSIDE** - fenced in area in back yard, no hazards.

There were no progress reports for the 3 school age children enrolled in the program. The other 4 children had a complete and up to date progress report.
All of the required items were observed in the first aid kit. However, Educator needs to add the school age children’s emergency contact forms.

Licensor reviewed 4 of the children's records. Of these, 3 were complete and up to date, but the 4th had an out of date physical and immunizations.
The blankets/sleeping bags that are used for napping are stored together. Discussed that these need to be stored separately to avoid cross contamination.
The children’s emergency contact forms in the first aid kit need to be updated. All of the required items were observed in the first aid kit. However, the CPR mouth guard is open and dirty and should be replaced.

Educator's first aid/CPR expired in 09/2021. Discussed that Educator needs to become recertified as soon as possible.
This monitoring visit inspection recorded no violations or advisories.
During the virtual visit, Licensor inspected all of the licensed space and observed the following hazards: ** Kitchen/Diningroom - cabinet under the sink isn't working all the time, and there are cleaners inside ** Livingroom - disinfectant wipes at the sign in station are accessible to the children ** Bedroom 1 - no hazards ** Bedroom 2 - no hazards ** Bathroom - no hazards ** Basement Playroom - no hazards

Licensor reviewed 3 of the children's records during the virtual visit. Of these, 2 were complete and up to date, the 3rd record had out of date signatures. Please review all of the children's records and ensure they are all complete and up to date.
Educator's first aid kit needs updated emergency contact forms, but all of the other required items were observed during the virtual visit.
In the morning, Educator waits out front at the road for the school bus and has a 2 year old with her. Then in the afternoons, Educator again waits out front for the school bus, but she has 3 young children with her. Educator resides on a busy street and has the children right at the roads during these 2 times every day. Discussed that the children cannot be outside at the road.
Toothbrushes are stored in a container together. They need to be stored in a manner in which they do not touch each other.
In walking to the back door in the basement, there was laundry detergent, paint cans, and other hazards on the way to the door. All other exits were clearly marked and easily accessible.
There was one outlet in the dining area that did not have a cover. There were also 2 outlets (1 in the livingroom and 1 in the hallway), that did not have an outlet plate covering the area around the outlets.

Licensor inspected all of the licensed, indoor space during the renewal visit. The following hazards were observed: ** KITCHEN/DININGROOM - outlet not covered. ** LIVINGROOM - outlet does not have a plate over it, surge protector has outlets not covered ** HALLWAY - another outlet that does not have a plate ** BEDROOM 2 - no hazards ** BATHROOM - no hazards ** BASEMENT: DAYCARE ROOM - no hazards.
Educator did not have any training certificates from 2021 or 2022. Licensor discussed with Educator that regulations state 10 hours of professional development have to be completed every year, for a total of 30 hours.
Educator has a detailed emergency preparedness plan. However, a plan for what to do if a child goes missing from the program needs to be added.
Licensor reviewed several of the children's progress reports and they were all overdue. Please ensure that all of the children's progress reports are up to date.
Licensor did not observe any immediate health risks during the monitoring visit. All of the required items were observed in the first aid kit. Licensor tested the smoke detector in the hallway and it worked appropriately. Educator had a carbon monoxide detector in the basement, but did not have one on the first floor. Discussed that regulations state a carbon monoxide detector needs to be located on every floor of the childcare home.
