A review of staff meetings indicated the following: -The program did not have documentation of completing staff meetings from September 2024 to August 2025. All licensees must hold monthly two-hour staff meetings with all members of the education team.
A review of children's records indicated the following: -One did not have a current child custody agreement in their file.
A child left the licensed space and walked into the adjoining hallway near exit. None of the educators were aware the child left the space. The child briefly looked around for something. The Site Coordinator then noticed the child. The child then went back into the licensed space on their own.
Although the staff schedule was posted at the program it did not have documentation of the current staff working at the program. The staff schedule must be current and posted at all times.
The program did not have a current building inspection certificate from the local Building Department as required.
A review of the emergency evacuation drills indicated the following: -The licensee did not have documentation of completing evacuation drills during the months of November and December 2025. Evacuation drills must be completed monthly with all groups of children.
This investigation inspection recorded no violations or advisories.

A review of educator's files indicated the following: -One did not register annually with the Department of Early Education and Care. -One did not have a current physical examination.

The licensee did not have observation of educators while working with children at least every two months by a staff with lead teacher, site coordinator or higher qualifications; 2. consultation with educators regarding children’s individual needs and communication with families; and 3. documentation of all observations and consultations.
A review of the emergency evacuation drills indicated the following: -The licensee did not have documentation of an emergency drill being completed in December 2023, January 2023-June 2023 no drills, November 2022 and December 2022 -The emergency evacuation drill for January 2024 did not have documentation of the length of the drill.
A review of medications at the program indicated the following: -Two children had current epi pens at the program however there was no paperwork attached.

Although educators were knowledgeable about the children's IHCP's; a review of the children/'s IHCP's indicated that one did not have documentation of the side effects of the medication, consequences of medication not given, and directions for storage of the medication.

A review of educator records indicated the following: -One did not have a current annual physical examination within one year prior to employment. -One did not have evidence of having been immunized in accordance with the recommendations of the Department of Public Health.
On the day of the visit, a child left the gym/cafeteria, unsupervised to an entryway area with an open exit door to the outside. The child was unsupervised for approximately 3 minutes. The Licensor informed the Site Coordinator of the unsupervised child in the entryway of the school.
The program did not have a current Building and Fire Inspection Certificate. Every program must provide a certificate of inspection from the local building and fire inspector certifying the facility is in compliance with all codes.

On the day of the visit the licensee had a Site Coordinator and three Group Leader Assistants assigned to school age children. According to EEC regulations a Site Coordinator, Group Leader, and Assistant Group Leader.
A review of five educators records indicated the following: -Two did not have evidence of having had a physical examination within one year. -Two did not have evidence of having been immunized in accordance with the recommendations of the Department of Public Health. -One did not have a signature and date on the staff orientation training form.
