Safe sleep
The licensor observed that the diaper changing table surface had tears in it, making it not intact and impervious to water. Additionally the diaper-changing surface did not have a covering to prevent the child from coming into contact with the diaper-changing surface.
The following hazards were observed in the indoor licensed space: there were diaper creams and wipes accessible, there were plastic bags from the Nerf darts the children opened on the floor that were choking hazards, there were several latex balloons accessible and books in plastic bags next to the pack and play were the infant naps. There was an open packet of Emergen-C 1,000mg Vitamin C Daily Immune Support accessible on the kitchen counter.
Supervision
The licensors observed the following regarding safe sleep: When the licensors arrived at the door, they heard an infant crying, the Educator opened the door, greeted the licensors, and then went into the room where the infant was in the pack and play for a nap. The licensor observed that the Educator had a mattress foam memory topper folded in half, inside that there was the pack-and-play mattress, and another blanket, and a tight fitted sheet was placed on top of all of that. The licensor asked the Educator to tell her about what she was using inside the pack-and-play, the Educator responded with" I did it to make it soft for the baby because the mattress is too hard." She went on to state that " As long as it had a tight fitted sheet it was fine". The licensor explained that infants need a surface that is firm and the Educator stated again that she thought infants needed a soft surface. This is especially concerning since the Educator was cited in March 2018 and June 2018 for safe sleep and has been required to retake the EEC Safe Sleep training multiple times. The fitted sheet did not fit properly leaving gaps at each end of the pack-and-play. The infant in care is 6 months old. Additionally the licensors observed several plastic bags with books inside them next to the pack-and-play. The licensors addressed this as a hazard and the Educator stated that is was not because the infant was unable to stand up and access them.

The Educator did not notify EEC regarding a change in her household composition. The licensors observed a person listed as regularly on the premises is now a household member. The Educator was unclear about how long ago the household member moved in, she initially stated 7 months and then changed it to 2 months. The Educator was cited in September 2023 during a monitoring visit for this person being on the premises without an approved BRC. The Educator was also cited in 2017 and 2018 for having household members move into the home without notifying EEC and having approved background checks.
The licensor observed 5 different pieces of outdoor equipment that were above 18 inches and did not have the required ground surface covering. There were 2 free-standing slides, one red one and one yellow one, there were 2 climbers that had slides attached, and 1 free- standing climber. The Educator was cited in 2018 for having a climber that was above 18 inches without the appropriate ground surface covering.
The licensors observed the Educator place a (REDACTED) seat and then start to feed her a bottle. The licensor reminded the Educator that infants must be held while bottle-fed and asked the Educator to pick up the infant.
The following hazards were observed in the backyard space: the water table had a broken piece on one edge of it, there was a pile of bricks that were accessible, and gardening tools were accessible there was standing rain water in the water table and chipping, peeling paint on the shed ramp. A child opened a bench outside and plastic bags were observed inside the bench.
A review of the children's records showed that one child has an identified allergy on her medical form. The Educator did not have the required confidential allergy list posted. The Educator was cited for this in February 2022.
The following supervision concerns were observed during the visit: the licensor observed that a child in care was eating the powder from an Emergen_C 1,000mg Vitamin C daily immune support supplement that was left on the kitchen counter accessible to him, a child was observed to have the end of a latex balloon in her mouth, a child picked up the infants bottle, which the Educator left on the top of the indoor climbing stairs, and was attempting to feed it to(REDACTED), the Educator sent(REDACTED) child inside the home to get a toy for the infant while the Educator was outside, the Educator allowed the children to play in a water table filled with rain water and two of the children were observed to put the rainwater in their mouths, swish it around and then spit it out, a child was observed to take a large yellow bag and place it over her head, the children were observed opening plastic bags of Nerf darts and putting the darts in their mouths and the plastic was left on the ground, which is a choking hazard and the licensor removed a small blue bead from a toddlers hand that the toddler found on the ground.
The licensor observed that the cabinet under the kitchen sink was not locked today and there were several cleaning supplies accessible

Supervision
The educator has had a person regularly on the premises since June who does not have an approved BRC from EEC. Initially the Educator stated that this person lives in the home, then changed it to this person is on the premises at least 3 times a week at 10am and in the afternoon between 2:30-3:00pm. The Educator does have a change of household transaction submitted since July
When the licensor arrived she observed a (REDACTED) year old outside in the backyard by herself, the educator was inside the home with a (REDACTED) year old.
The licensor did not observe a gate at the bottom of the stairs from the living room
The licensor observed that the cabinet under the kitchen sink was not locked and there were several cleaning supplies accessible

The licensor observed some books, crayons, blocks and a few other small toys accessible. There was a shelf covered by a blanket that contained some toys but they were not accessible to the children. The educator stated that they were covered because she was having a problem with ants in that area. The Educator has been cited for this 4 times since 2018. The outdoor space had child appropriate toys/materials
The Educator reported that she is having an issue with ants in the child care space. She stated that she has tried to remove them but they keep coming back in this one area. There are child care toys that are in that area that the children do not have access to due to this
The licensor observed chipping/peeling paint in the main play area of the home. The baseboard and the doorway had peeling paint
There are not enough materials in the space to support a developmentally appropriate curriculum for the children in care.

The Educator is using porta cribs for children who are over three years old. Discussed that older children should be napping on mats, and should not be in porta cribs.
Records were reviewed for all enrolled children. Two children did not have complete enrollment packets (only the odd numbered pages were in the files; the even numbered pages were missing) One child did not have proof of current physical and immunizations.

Equipment and Materials
There was one child with documented allergies and with a life saving medication. This was not posted as required by EEC regulations.
The EEC Parent Handbook the Educator was using was missing 4 pages. EEC Licensor agreed to email the EEC Parent Handbook to the Educator and gave technical assistance about filling out the required information in the handbook.
There were cleaning products and chemicals accessible to the children under the bathroom sink.
There is not a sufficient amount of developmentally appropriate materials accessible to the children. This is something the Educator has been cited for previously.
The educator has a documented fire drill on 8/17/19 at 11am, which is a Saturday. There were no children in care on that day.
The first aid kit did not contain the emergency contact cards for the children in care
The licensor observed puzzles, books, a kitchen set(with no accessories) to be accessible to the children in the main child care area. The rest of the toys/materials were stacked inside 2 pack and plays so the licensor was unable to assess if they were appropriate to the children in care. The educator was cited for this on the previous visit
