Progress Reports
Current consent for administered medications
Written progress reports are not being completed as required by EEC. Evaluations must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older). Parents must receive a copy, and a copy must be kept in the child's file at the program.
One school age child receives a [REDACTED] The mother brings in [REDACTED] since the child does not attend the program every day. The educator stated she places the pill bottle on the top of the refrigerator, so it is out of reach of the children. The educator has a written log she uses anytime she gives the child the medication. However, the parent has not signed the EEC required medication administration consent form.

Ages and stages questionnaires and EEC school age reports reviewed in children's files were last completed in March, April and May of 2025. One child had not had an assessment done since the summer of 2024. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).
Two children have allergies listed on their medical paperwork in their files. One child has an EpiPen, the other child, whose paperwork listed nut allergies, does not. Neither child has written medication consent forms signed by the parent or Individual Health Care Plans signed by their pediatrician. These forms must be kept on file at the program and updated every year. The child with the nut allergy must have the required paperwork (and EpiPen) on site at the program or a letter from their doctor stating that the child does not have life threatening nut allergies that require lifesaving medications.

Educator was missing a carbon monoxide detector. Discussed purchasing plug in detectors.
Educator has an allergy list posted on the inside of a kitchen cabinet. One child has an [REDACTED]. Discussed adding the location of the EpiPen to the list. [REDACTED] goes back and forth between the public school, home and the program. The child must have an [REDACTED] that stays at the program and is located in the prescription box. EpiPen must be stored where it is easy for the educator to access it, but out of reach of the children.

School age child with an allergy and an [REDACTED]. Allergy list was not posted as required. Child did not have an Individual Health Care Plan completed by the pediatrician. Parent has not signed the EEC Medication Consent form. These IHCP and consent forms must be kept on file at the program. The EpiPen must be stored in a location that is readily accessible to the child, but not accessible to the other children in the program.
This monitoring visit inspection recorded no violations or advisories.
