At the time of the visit small pom poms were inside of a sensory table in the Toddler 2 classroom. They were small enough to be considered a choking hazard.
The gate in the Toddler 1 classroom was not secured.
Fire drills were not documented for 2/2024 and 9/2024.
A child in the PS 1 classroom was observed using the toilet and not washing his hands after.
The diaper changing pad was torn so it was not impervious to bodily fluids.

At the time of the visit the following was noted: 1. Chipping/peeling paint was found in Infant, Toddler 2 and PS classrooms. 2. A hole was found in a wall in the PS classroom.
