Several maintenance and safety issues within the facility were observed during the visit: 1. Preschool Classroom: • Broken/cracked wall. • Floors need cleaning, but repeated attempts haven’t improved the appearance. • Rusted heater baseboard. 2. Hallway Area: • Multiple cracked walls. 3. Rug Issue: • Rug coming up and taped to the floor, posing a tripping hazard. 4. Pre-K Classroom: • Bathroom ceiling has chipping/peeling paint. 5. Main Entrance: • Hole in the wall with exposed cables and pipes. • Ceiling with leak stains. 6. Infant classroom: • Draft from a broken window that cannot be opened or closed making it difficult to regulate the room temperature. • Cracking and peeling paint on the ceiling. • There is an issue with the hot water in the infant classroom. 7. Toddler Classroom: • Broken window. • Chipping/peeling paint on the baseboards around the windows.
1- In the preschool classroom there is a heater with a rusted baseboard. 2- In the infant classroom there is a draft from a broken window that cannot be opened or closed making it difficult to regulate the room temperature.

Individual Health Care Plans were missing information on who was trained on the medical condition and what staff were trained.
Staff files were missing current staff evaluations, staff observations and BRC information.
The program administrators could not locate the first aid cards at the time of the visit.

Diapering and Toileting for children under 2.9
Children's enrollment forms were expired, physicals were expired and one child did not have proof of Lead.
BRC information was missing from staff files.
In the infant room the heater end cap had shifted creating a space for the children's fingers to get stuck, in the preschool bathroom there was a storage cabinet that had an open hole that was sharp.
The infant room and younger preschool room had shelves that had chipping paint.
There were missing fire drills from May and June of 2019.
