Individual Health Care Plan (IHCP)
The Individual Health Care plan (IHCP) for one child with a medical condition had an expiration date of 8/15/25.
The program did not have current written parental consent for the administration of medication required by an IHCP.

Supervision of children
On the day of the visit Educators did not know how many children they were responsible for. One Educators reported having 16 children but upon counting it was learned there were 25 children in the group

Review of the children’s records revealed that files did not include emergency medical consent
There was an assistant group leader alone, assigned to supervise a group of 12 school age children
The program didn’t have parental concert for the administration of rescue medication required by an IHCP
Review of children’s records revealed that an IHCP was missing parent signature, names of staff who received training in administering the treatment.
On the day of the visit Educators in 2 groups did not have a method of knowing which children were in their groups.

Licensor observed a group leader leave a group of children alone in a room while they went to an adjoining room to get plying cards.
Random review of children’s files revealed ant two children with chronic medical conditions and with medication did not have parental consent for the Administration of the medication required by the IHCP.

A review of the attendance records revealed that a child, in a drop-in slot, was not listed on the attendance as being present. In addition, there were 2 children listed on the attendance as being present, but the children had already left for the day and not signed out.
This enhanced monitoring visit inspection recorded no violations or advisories.
Review of children’s records revealed that one file contained an IHCP and Medication Authorization form that expired on 1/28/20.

On the day of the virtual visit and through review of the staff records checklist it was learned that there were two Educators that did not hold current first aid certification.
On the day of the visit the first aid kit did not include medical tape and tweezers.

The Program was not documenting children’s arrival times. In addition 50 children went out side with educators. One educator remained inside with one child with the attendance sheet so the educators with the group of 50 children did not have a method of knowing which children were on the poremese. The attendance record for kindergarten children did not include a (REDACTED) that was in the group. Educator reported that the third grader was signed into the attendance record for (REDACTED) group. Making both attendance records inaccurate.

Evacuation procedures posted at each exit
On the day of the visit there was a board that included the names of staff and the groups they were responsible for. The board did not include information on the qualifications of each Educators or their positions. It also did not include information regarding first aid and CPR certification for each staff.
On the day of the visit the program did not have a snack menu available. One Educator reported that the snack is listed each day on the white board that includes the daily schedule. The snack information on the board is erased each day at the end for the program day so the program is not maintaining past menus.
On the day of the visit the children were allowed to use the climbing structures and swings while the ground was frozen
Emergency evacuation procedures were not posted near the exit in the cafeteria where children have snack. There was no diagram near exits that did not lead directly outdoors.
